• Birth certificates are confidential documents; therefore, only the individual, the biological or adoptive parents of the individual, the biological or adoptive grandparents of the individual, an adult sibling, the guardian or temporary guardian of the individual, a legal spouse, personal representative of the individual’s estate, or an adult child of the individual can request and obtain the birth certificate.
• If you were born anywhere in the State of Georgia, you may request a certified copy of your birth certificate from the Probate Court of Pike County. 
• Vital Records requests are fulfilled in the office any time during regular operating hours and can be made in person, by phone, or via fax/email. 

•  Valid photo ID. To obtain a birth certificate you must present a Georgia Driver's license or State of Georgia Identification Card unexpired or expired for not more than one year, State of Georgia Weapons Carry License, unexpired Driver's License or Identification Card issued by another U.S. State, jurisdiction or territory, unexpired U.S. Passport, unexpired Foreign Passport, U.S. Military Identification, Military Dependent Identification, Veteran's Identification, unexpired Consulate Card, Transportation ID, Debit Card with Picture and Employer ID Card. 
• Payment.  The cost is $25 for a certified copy and $5 for each additional copy received at the same time. The Court accepts cash, debit or credit cards (user convenience fee applied), money orders (exact amount) or personal check for fees associated with vital records. 

Yes. For requests via mail the Probate Court will need the Vital Records Request Form below submitted along with a form of ID (specified above). If you would like the copies to be mailed to you, you must provide a self-addressed, stamped envelope.
